One 2026 implementation guide reports that hospitals acting as Health Information Providers under the Ayushman Bharat Digital Mission keep records at the hospital and share them only with explicit, time-limited and revocable patient consent.

The building blocks

  • ABHA, the 14-digit health ID.
  • The Health Facility Registry and Health Professional Registry.
  • FHIR R4 as the required data format.
  • Three certification milestones, from ABHA creation to consent-based exchange of encrypted records.

The guide says the hardest work is mapping a hospital's own data layout to FHIR. It also says NABH's 6th edition, effective 1 January 2025, puts more weight on digital records. The source is a vendor blog, so verify each claim with ABDM and NABH before you commit.

For buyers

Ask a software vendor which milestones they have completed, on which hospitals, and for a test log. Insist on audit logs and encryption in the contract.
